You may experience delays in scheduling appointments, accessing your medical records, or receiving certain services. Rest assured, we are working diligently to ensure the transition is as smooth and seamless as possible.

Below is a link to the patient FAQ for our new medical record system. This is just for reference and there is nothing you need to do at this time. When we begin using the new system you will receive a welcome message/email with directions for accessing your records through the system, known as the “Elation Health Patient Passport”.
